A L L E N W A L K E R PATIENT CHART → NAME: WALKER, Allen [INFORMATION UNCONFIRMED] GENDER: ♂ D.O.B: xx/xx/xxxx [INFORMATION UNCONFIRMED] NATIONALITY: BRITISH [INFORMATION UNCONFIRMED] ADDRESS: [INFORMATION UNCONFIRMED] ___________________________ ___________________________ ___________________________ HEIGHT/WEIGHT: 169CM/55kg BLOOD TYPE: O NEXT OF KIN: WALKER, Mana [INFORMATION UNCONFIRMED] EXTENDED REPORT → Patient was admitted to Princeton-Plainsboro Teaching Hospital in the early hours of the morning of 08/16/08 with multiple broken bones and aberrations, suspected to be the result of a fall from a two story building. Patient is also suffering from tachycardia, delusions, and low potassium. Patient seems to also be suffering from extreme amnesia as he expresses no knowledge as to how he fell from the roof nor being the state of New Jersey. The patient in question, WALKER, Allen, has also also not found in the database. The two names we have been given by him as means of emergency contact, WALKER, Mana and LEE, Komui, were also not found on the database. Patient also displays premature whitening of the hair suspected to be due to melanin reduction, however the cause is currently unknown. In addition to this patient also has an unnatural scarring below, across and above his left eye suspected to be the result of abuse or self-harm. However, this too, is unconfirmed. Patient's left arm also displays unusual and peculiar anomalies.
